Online Entry Form

Please note that this form will pre-enter you into the computer but you will
STILL NEED TO BRING A SIGNED ENTRY FORM TO THE SHOW!

Note: Please Do Not Enter Forms After 8PM 1 Day Prior To The Shows

USEF numbers are required for USEF shows!

To enter, follow these simple steps:

1. Use the PDF form on the specific show's page on this website OR click on the form below to open it.
2. Fill out all applicable fields.
3. If there is a message in the purple banner across the top to "Sign" the form, please follow the directions to sign the form.
4. Save the form to your computer and remember WHERE you saved it (for example, C:/My Documents/cinnbayinc_entry_form.pdf).
5. Send an email to Charlene at Cinnbayinc@aol.com and attach the cinnbayinc_entry_form.pdf to the email.
6. Print the form, sign it, and bring it with you to the show!

You can now re-use this form multiple times. You can also print and FAX this form to Charlene at 401-848-2985.
